Preliminary Requirements:

    • Submit updated resume' to (pdf or word file) 
    • Complete the online application
      • Be sure to provide a valid email address for all three professional references on your application. EOC requires professional reference recommendations prior to making a final hiring decision. 
      • Interviews will only be confirmed for fully completed applications.

Prior to Interview:

  • Complete an Arrest/Conviction Report and Certification Form (6004)
  • Complete a Sexual Misconduct/Abuse Disclosure form for every current and previous employer in which you had contact with children. (According to state legislation, a separate form must be filled out for each employer.)
    • Complete section 1 of the form—including the required signature on page 2—for each employer. You may, as a result, have several forms to submit.
    • You are only eligible for employment if EOC receives forms back from all of the employers you list and no record of misconduct is revealed. Therefore, we strongly encourage you to complete these forms now, in anticipation of potential employment and return them to as soon as possible.

After Interview:

After an interview is completed, all three references provided on your application are sent an email requesting feedback. Once reference feedback is reviewed a final hiring decision is made.

Final Documents:

After an offer of employment is extended all of the following documents must be submitted to finalize employment.  We encourage all candidates to submit these documents during the preliminary stage to expedite the process.

  1. Proof of a Bachelor's Degree from an accredited University or College (Transcripts reflecting degree and date conferred) 
  2. Proof of Teacher Certification and/or other certifications if applicable
  3. Required Online Training (submit the certificate of completion for each training)
    • Act 31: Recognizing and Reporting Child Abuse Training
    • Act 71: Society for the Prevention of Teen Suicide training
    • Act 126: Professional Ethics and the Educator Discipline Ac
  4. Proof of a TB test or chest x-ray that is less than one year old. Click Here for a general School Personnel Health form to be completed by a physician. (we do not need a full physical) 
  5. Pennsylvania Child Abuse History Clearance (Act 151) that is clean and less than one year old

  6. State Criminal Record Check from the Pennsylvania State Police (PATCH - Act 34)  that is clean and less than one year old.

  1. PA Dept of Education FBI Criminal Background Check (Act 14) that is clean and less than one year old.
    • Cost: $22.60 (paid at the time of fingerprinting

    • Opt for a walk-in if there are no appointments within the week of registering to avoid any hiring delays.
    • Within 7 business days of processing your fingerprints, you will get an email from PASafeCheck <> with a one-time access link to your clearance letter.
